Positive behavior supports

Positive Behavior Supports are used when there is a need to develop a plan which identifies prevention strategies to reduce significant challenging behaviors which interfere with activities of daily living, social interaction, or work. Our behavior specialists will complete a Functional Assessment, create a Positive Behavior Support Plan, instruct on evidence-based replacement skills and new ways to respond to challenging behaviors.

Respite is a short-term support that is provided in the absence of, or to give relief to, any individual providing care to the participant. It is available for a person who does not receive residential services and resides in his/her own home or family's home and is unable to independently administer self-care. Respite may be provided in the home or the community.

Residential Support, Level II

Residential Support, Level II is provided in a family home provider setting, for no more than three people living together who require up to 24 hours a day of support. Commonwealth Connections will support individuals seeking a Family Home Provider setting to live in. This support is focused around matching an individual with a Family Home Provider based on needs and in order to provide the participant with a loving, safe home and family setting.

Person-Centered Coaching is available to a participant when a barrier challenges the success of the person to meet their goals or when there is a barrier for the successful implementation of the Plan of Care (POC). The person-centered coach works under the direction of a positive behavior support specialist or other licensed professional on the POC.

Community living supports

Community Living Supports (CLS) will be provided to facilitate independence and promote integration into the community for a participant residing in her/her own home or in his/her family's home. CLS will be supports and assistance related to plan of care goals and may include: routine household tasks, daily living skills, hygiene tasks, shopping/money management, socialization, relationship building, leisure choices, and/or participation in community activities or therapeutic goals.
